In person at Evergreen Counseling & Mediation. A form will be provided for you to complete, sign, and return.

By submitting your consent and phone number, you are authorizing Evergreen Counseling & Mediation to send you SMS messages and SMS notifications.

Opt-Out Instructions
You may opt out of receiving messages at any time by replying “STOP” to any message. Once you opt out, you will no longer receive SMS communications from Evergreen.
